One of the most frustrating things about traumatic brain injury is that the evidence is not always obvious.
A person can walk into a room, hold a conversation, and have a normal MRI — yet still be struggling with changes that impact their ability to work, communicate, and manage daily life.
This is where many people misunderstand brain injury.
They assume that if something cannot be seen on imaging, it cannot be significant.
But the brain does more than show up on a scan.
It allows us to process information, remember conversations, make decisions, organize our day, communicate effectively, and adapt when situations change.
And those abilities can be affected even when traditional imaging does not tell the full story.
The Question We Often Ask Is Not Always the Right One
In many TBI cases, the first question becomes:
“Is there visible damage?”
That question matters.
But it is only part of the picture.
A more complete question is:
“What changed after the injury?”
Because the impact of a brain injury is often seen in function.
Can the person still manage the responsibilities they had before?
Can they process complex information the same way?
Can they participate in conversations, make decisions, and complete tasks without the same level of difficulty?
Those are the questions that often determine how an injury affects someone’s life.
The Difference Between What We See and What We Experience
A useful way to understand this is the difference between hardware and software.
Imaging helps us evaluate the hardware — the physical structure of the brain.
But daily functioning depends on the software — the systems that allow the brain to process, organize, communicate, and respond.
A computer can look intact from the outside while still having performance issues.
The same can happen with the brain.
Someone may appear fine during a brief interaction but struggle when they are asked to manage multiple pieces of information, solve a problem, or communicate under pressure.
Why Cognitive Communication Evaluations Matter
Cognitive communication evaluations examine the skills that allow people to function in real-world situations.
This includes areas such as attention, memory, executive functioning, and communication.
The goal is not simply to identify whether someone has difficulty.
The goal is to understand how those difficulties affect daily performance.
For example:
Can they follow complex conversations?
Can they remember important information after appointments?
Can they organize tasks independently?
Can they communicate effectively when stressed or overwhelmed?
These are the skills that influence independence, employment, and quality of life.
Why This Matters in Legal and Forensic Settings
In litigation, the question is often not simply whether an injury exists.
The question is how that injury changed the person’s ability to function.
That requires looking beyond a single data point.
Medical records, reported symptoms, baseline abilities, functional changes, and cognitive communication findings all contribute to understanding the full picture.
Because a brain injury is not defined only by what appears on an image.
It is defined by how the person’s life has changed.
The Bottom Line
A normal MRI can provide valuable information.
But it does not always explain the entire story.
When the imaging and the person’s experience do not match, the answer is not to ignore the difference.
The answer is to ask better questions.
What changed?
What tasks became harder?
What abilities were affected?
And what evidence helps explain that change?
